The Kentucky Oral Health Coalition (KOHC) is working to improve oral health in Kentucky by addressing access to care, increasing school based programming, and improving oral health literacy. KOHC has brought together many stakeholders to create a strong oral health network designed to collaboratively make progress in the state’s oral health outcomes. Throughout the work, we have been consistently impressed with the “bright spots” in oral health located in every county across the state. These champions of oral health seek to collectively improve the health of every resident.

This “Bright Spots” project will lift up promising replicable practices and create a stronger statewide network. Bright Spots are organizations, projects, coalitions, schools, programs, models, or initiatives that target, address, and/or invest in key oral health priorities for people in Kentucky of every age group, geographic location, and race / ethnicity. The initiative also seeks to leverage these Bright Spots to encourage collaboration between stakeholders focused on similar issues in sharing data-driven approaches, promising practices, peer advice, and effective partnerships, ultimately resulting in improved health outcomes in Kentucky.

Bright Spots Criteria
Bright Spots will be selected and recognized based on the following criteria:
Target: Is the target of programming low-income children and adults?
Focus: Does the Bright Spot support key KOHC priorities: access to care; school based health; or oral health literacy?
Impact: Has the Bright Spot increased health outcomes for low-income residents?
Measurable: Does the Bright Spot demonstrate an evidence-based approach? Is the Bright Spot based on data-driven solutions? Are there measurable goals that evaluate the effectiveness of the Bright Spot? Does the proposed Bright Spot have data to prove its success and impact?

Nominations and Recognition
Each July, there is a call for nominations in 3 categories: School Based Oral Health, Access to Oral Health, and Oral Health Literacy. Nominations are gathered from the online nomination form located on the KOHC website: kyoralhealthcoalition.org. The KOHC Bright Spots Committee will open the process, advertise it, accept applications, review and select the annual bright spots. KOHC staff or committee members may contact those nominated with additional questions, to provide feedback and/or inform you whether the proposed Bright Spot has been selected. Each one will be presented with a certificate and a Bright Spot Case Study will be produced and placed on the website.

Thank you for nominating a Bright Spot in Oral Health (Bright Spot). Please provide responses to the questions below to the best of your ability. Please note that the information captured on this form may be made public. In order to fully consider your nomination, we would like to receive responses to all of the following questions, however, your response to each question is voluntary.

BRIGHT SPOT REQUIREMENTS
As you consider nominating Bright Spots, please ensure they address each of the following educational priorities and requirements before submitting your nomination:
-Serves Kentucky’s low-income community;
-Mission aligns with the KOHC’s key priorities: access to care; school based health; or oral health literacy
-Has measurable goals that evaluate its effectiveness and impact; and
-Demonstrates an evidence-based approach.

By submitting a Bright Spot in Oral Health (Bright Spot) nomination (which includes the associated nomination form), the entrant (Nominator) agrees to the Nomination guidelines and these terms and conditions:

Number of Entries: There is no limit to the number of entries submitted per person.

Finalist Selection and Notification: Nominations are reviewed by the KOHC Bright Spots Committee and staff. The decisions of the committee on all matters relating to the Bright Spots are final. The Initiative will not consider any nomination that does not adhere to these guidelines. Selected Bright Spots will be notified via email and/or phone call.

Copyright/Original Work: Each Nominator represents and warrants that he or she is the sole author and owner of the submitted materials (including the nomination) that represents the Bright Spot; that the nomination is wholly original to the Nominator (or is an improved version of an existing work that the Nominator has sufficient rights to use and to authorize others (including KOHC) to use); and that the Nominator does not infringe any copyright or any other rights of any third party.

Intellectual Property Rights: By providing a Bright Spot and nomination to KOHC, each Nominator grants to KOHC an irrevocable, paid-up, royalty-free, non-exclusive worldwide license to use, copy, distribute to the public, create derivative works from, link to, and display publicly (on the Internet or otherwise) the submitted Bright Spot (including the associated nomination materials). All Nominators will retain all other intellectual property rights over themselves and the associated nomination materials. Each accepted Nominator acknowledges that the nomination will be provided to the public free of charge by KOHC or its designee starting on the date of the announcement of accepted Bright Spots. Each selected Nominator also agrees to allow KOHC to use his or her name (and/or organization’s name) and other submitted information (i.e., address) in any news report, media release, or documentation issued by KPHC (or its designee) related to the Bright Spots in Oral Health.

Limitation of Liability: By submitting an entry as a Bright Spot, all Nominators agree to, and thereby do, release, discharge, and hold harmless KOHC and its employees, agents, contractors, and representatives (except in the case of willful misconduct) from any claims, losses, and damages arising out of their participation in the Bright Spot or any Bright Spots-related activities and the acceptance and use, misuse, or possession of any prize awarded hereunder, including claims for injury, death, damage, or loss of property, revenue or profits, whether direct, indirect, or consequential, arising from their participation in the Bright Spots nomination, whether the injury, death, damage or loss arises through negligence, or otherwise. Nominators will not be required to waive claims against KOHC that arise from the unauthorized use or disclosure by the agency of the intellectual property, trade secrets, or confidential information of the Nominator. The Nominator shall be liable for, and shall indemnify and hold harmless KOHC against, all actions or claims for loss of or damage to property resulting from the fault, negligence, or wrongful act or omission of the Nominator.

Privacy Policy: Participation in Bright Spots constitutes the Nominator’s consent to the use by KOHC (and its agents and contractors) of the Nominator’s name, likeness, photograph, voice, opinions, and contact information (i.e., email, phone number, and address) for communication purposes on matters regarding the Bright Spot in any media, worldwide, without payment or consideration. Participation as a Nominator also constitutes the Nominator’s consent to permit KOHC and its agents and contractors to share any nomination information of the Bright Spot made by the Nominator. Individually identifiable information provided to KOHC by submitting a nomination in Bright Spots will be used only to communicate on matters regarding the Nominator’s submission or administering or implementing the Bright Spot initiative. Individually identifiable information provided to KOHC by submitting a nomination will not be used for any purpose that is unrelated to the submission and/or administering or implementing the Bright Spot initiative, unless such a use is required by law.
